เอกสารสำหรับใช้ในการแจ้งขอมรณบัตร

• มรณบัตรของทางการอินเดีย โดยผ่านการรับรองจากทางการอินเดีย
• หนังสือเดินทางของผู้แจ้งขอมรณบัตรและผู้ถึงแก่กรรม
• บัตรประจำตัวประชาชนหรือทะเบียนบ้านของผู้แจ้งและผู้ถึงแก่กรรม
• ใบรับรองการเผาศพหรือจัดการศพของผู้เสียชีวิต

กรุณายื่นคำร้องขอมรณบัตร ที่ฝ่ายกงสุล สถานกงสุลใหญ่ ณ เมืองมุมไบ ระหว่างเวลา 09.30 – 12.30 น. ในวันทำการ

Document required to apply for Thai Death Certificate

1. Thai death certificate application form which the applicant can acquire and
   submit at the Royal Thai Consulate-General in Mumbai, Consular Section
2. Death Certificate issued by the Indian Authorities concerned and legalized by Mantralaya
3. Passport of the applicant and death person
4. Thai ID card and / or House Registration document of the applicant and the
   death person
5. Cremation Certificate
6. No service fee is required whatsoever

Please contact Consul Section at the Royal Thai Consulate-General in Mumbai between 09.30 - 12.30 hrs. on a working day.
